Krazy Kids Consent Form

Allergy Needs

Please could you inform us whether your child is allergic to any item. This includes milk powder and possible traces of nuts as well as other foods to which your child may be allergic.

Please note that it is the responsibility of the parent / carer to keep Krazy Kids informed of any allergies that your child may have now or in the future.

Breakfast Cereal

Certain breakfast cereals have been requested by the children which we can only provide with your consent. This is due to healthy eating regulations set out by the government. We already provide Weetabix, Cheerios, Rice Crispies, Special K, Cornflakes and, occasionally, Coco Pops.

Complaints

We follow the same Complaints Procedure as the school, the policy for which can be found on www.bureparkprimary.org under Policies.

Transport

Please indicate whether you give permission for staff at Krazy Kids to take your child (named above) on outings to the local park, nature reserve or shop whilst in their care*

Sunscreen

Please indicate whether you give permission for staff at Krazy Kids to apply sunscreen supplied by you for your child (named above)*

Internet Access

Please indicate whether you give permission for your child to use Krazy Kids devices to access the internet, via the school's secure provision, as an activity at Krazy Kids*

Photos

Please indicate whether you give permission for staff at Krazy Kids to take photos of your child (named above) for displays or the school website*
